IMPORTANT: Each camper’s immunization record is required with registration form and must be included in the binder due upon arrival.

Designate an adult with a cell phone to contact when a student fails to show for medication. 2018 Church Medication Contact Sheet

Every adult must have a background check. Permission must be obtained before running a background check. Use your own form or this form. Form Background Consent


Every adult must take the Child Protection Training.

The 4-Part Child Protection Training Videos can be accessed by pressing the start button on each of the videos below:

 Child Protection Training Manual to be used with the Training Videos. (If for any reason you are unable to view or show these videos via the links provided, please contact the SJBA office for a DVD to be mailed to your church).

 Once the videos have been reviewed by each adult, each adult must take the test: Child Protection Training Test

After taking the test, the church personnel administering the test must grade the test. (Please contact the SJBA office for the Answer Key). If the sponsor passes the test, then a completed copy of this certificate MUST be turned in to the SJBA Office to show the date of completion of the successfully completed training for each sponsor. Child Protection Certificate

IMPORTANT! – Texas Administrative Code, Title 25, Part 1 Chapter 265, Sub-chapter B requires an annual background check and a copy of the background check be turned in on arrival at camp. Background Checks Texas Admin Code

Bring the above forms in a 3-ring binder. See notes below.

  1. Please put your Student Medical Releases in a three ring binder in alphabetical order.  Please be sure all have signatures of both student and parent or legal guardian (If not, there will be a delay in campers’ starting their camp activities).
  1. Please put each Adult’s Medical Release, Child Protection Certificates and Background Check together and then place them in alphabetical order in the back of binder (behind students).  This makes for a quick reference for our nursing staff to access your student or sponsor’s file in case of emergency.
  1. Create a cover page which clearly displays your church name, number of students attending and number of adults attending(e.g. First Church, 275 students & 30 adults).  This completed binder is due immediately upon arrival with all forms properly filled out.
